A landmark new report from the Royal College of Nursing paints a stark picture of the school nursing workforce in England – and the implications for children’s health are serious. But within the data lies a clear opportunity: to make every school nurse contact count more than it ever has before.
The crisis in numbers
Published in May 2026, the RCN’s Prevention Starts Here report reveals that since 2015, the number of NHS-employed school nurses has declined by 29%. Today, the average school nurse in England is responsible for approximately 4,000 pupils – far beyond any reasonable safe caseload.
The survey findings from school nurses themselves make uncomfortable reading:

Only 23% say their service is fully staffed

73% believe workforce shortages are having a major negative impact on the support available to children and young people

56% say understaffing has reduced the time available for prevention and early intervention

50% report limited capacity to deliver health promotion activities including immunisation programmes
Behind every one of these statistics is a school nurse trying to prioritise an impossible caseload, and a child who may not get the timely support they need.
The shift away from prevention
One of the most troubling findings in the report is what school nurses are being forced to deprioritise. With safeguarding demands consuming the majority of their time, proactive prevention work – the very thing school nurses are uniquely trained and positioned to deliver – is increasingly falling through the cracks.
Immunisation programmes, for example, are among the least consistently delivered services: 50% of school nurses said immunisation is never addressed in their service. This is happening at a time when the UK has already lost its WHO measles elimination status.
The report is clear: school nurses should not be plugging gaps left by overstretched CAMHS and children’s social care. Their core value lies in prevention and early intervention. But to do that, they need to be freed from the administrative and logistical burden that currently consumes so much of their working day.
